REQUIREMENTS

ORIGINAL AND 3 COPIES EACH: 

  1. Authenticated PSA Birth Certificate (Legalized)
  2. Certificate of No Report of Marriage issued by PSA – CENOMAR (“Certificate or Proof of Singleness”) Legalized
  3. Three (3) duly filled out application forms
  4. Data Page of the Philippine Passport of the Groom and the Bride
  5. Residence Permit of the Groom and the Bride (back-to-back)
  6. Names of two (2) witnesses and copy each of their ID card or Passport

Applicant who is a widower:

  1. PSA Death Certificate of the former husband/wife 

For Applicants between 21-25 years old:

  1. Parental/Guardian consent (In the event that parental/guardian consent is not given, there is a waiting period of three (3) months after the bonds are posted)

For Applicants between 18-21 years old:

  1. Affidavit of parental Consent/Advice -  must be applied personally by the parents at the Consulate. 

APPLICATION PROCESS:

  1. Walk in to the Consulate (We are open Monday - Friday from 8:30am - 4:30pm except during declared PHL and ES public holidays)
  2. Present your documentation at Window 1 or 2 
  3. Pay at the Cashier (Window 3), payment by Cash, Debit/Credit cards and Bizum are accepted

REQUIREMENTS FOR THE PETITION FOR CORRECTION OF AN ENTRY IN THE VITAL EVENT (ROB/ROM/ROD)

ORIGINAL AND 4 COPIES EACH: 

  1. Application Form
  2. PSA Birth Certificate or PSA Marriage Certificate or PSA Death Certificate
  3. Valid Data Page of the Philippine Passport of the person who applies
  4. At least three (3) public documents showing the correct
  5. Other documents which the petitioner or the Consul General may consider relevant and necessary for the approval of the petition

REQUIREMENTS

ORIGINAL AND A COPY EACH, IF THE LOST PASSPORT IS VALID: 

  1. Affidavit of Loss to be executed at the Consulate
  2. Birth Certificate issued by PSA/NSO
  3. Marriage Certificate issued by PSA/NSO (if married and using the LAST NAME of the husband)
  4. Photocopy of the data page of the lost Passport
  5. Two (2) valid IDs issued by the Philippine Government
  6. Police Report (if possible in English)

REQUIREMENTS

  1. Personal appearance of the Applicant
  2. Completed Form
  3. Original and one (1) photocopy of the Data Page of the Philippine Passport
  4. Three (3) passport photos 4.5 cm x 3.5cm in size, taken in the last six (6) months with white background
  5. Non-refundable fee of 27€, payable in cash

Note: In case the Passport was lost/stolen, the applicant will have to apply for an Affidavit of Lost Passport and submit it for the Travel Document.
