ORIGINAL AND 4 COPIES EACH:

1. Report of Death Form
2. Certificado Literal de Defunción issued by Registro Civil
3. Certificado Plurilingue de Defunción issued by Registro Civil
4. Medical Death Certificate issued by OMC
5. Passport of Deceased and Photocopy of Data Page
6. If Dual Citizen, dual citizenship documents: Oath of Allegiance, Order of Approval or Identification Certificate

ADDITIONAL DOCUMENTS IF YOU WANT TO REPATRIATE THE MORTAL REMAINS OR ASHES (Present Original, 4 copies each and translation in English):

REPATRIATE MORTAL REMAINS:

1. Certificate of Embalming (Acta de Embalsamamiento)
2. Burial Permit issued by Registro Civil (Permiso de Sepultura)
3. Certificate of Sealing of Casket (Certificado de Precinto de ataúd)
4. Body Transit Permit (Autorización de salida del fallecido)
5. Health permit to transfer the deceased (Autorización de Traslado por la Consejería de Sanidad)
6. Confirmed flight Schedule 

REPATRIATE ASHES:

1. Cremation Certificate (Certificado de Incineración).
2. Sealing of the urn Certificate (Certificado de precinto de urna)
3. Passport of the bearer of the urn
4. Confirmed flight Schedule

APPLICATION PROCESS:

1. Walk in to the Consulate (We are open Monday - Friday from 8:30am - 4:30pm except during declared PHL and ES public holidays)
2. Present your documentation at Window 1 or 2
