- Applicants can bring their own affidavit or they can download a template here
- Photocopy of valid identification card (passport)
- Other documents that the applicant would like to attach
APPLICATION PROCESS:
- Walk in to the Consulate (We are open Monday - Friday from 8:30am - 4:30pm except during declared PHL and ES public holidays)
- Present your documentation at Window 1 or 2
- Pay at the Cashier (Window 3), payment by Cash, Debit/Credit cards and Bizum are accepted
