- Valid Philippine Passport
- Photocopy of PSA Birth Certificate
- Photocopy of PSA Marriage Certificate
- Photocopy of Finality of Annulment of Marriage (If needed)
PASSPORT
- Photocopy of the data page of passport (page with picture and details of the holder)
CERTIFIED TRUE COPY OF PASSPORT
- Photocopy of the data page of passport (page with picture and details of the holder)
CERTIFICATE OF REGISTRATION OF BIRTH
- Will be issued (if required) by the Consulate upon submission of the complete requirements for the registration of Report of Birth
APPLICATION PROCESS:
- Walk in to the Consulate (We are open Monday - Friday from 8:30am - 4:30pm except during declared PHL and ES public holidays)
- Present your documentation at Window 1 or 2
- Pay at the Cashier (Window 3), payment by Cash, Debit/Credit cards and Bizum are accepted
