REQUIREMENTS

ORIGINAL AND ONE (1) COPY EACH OF: 

1. Duly Accomplished Application Form
2. Valid Passport of parents (Data Page only)
3. Valid Residence Permit of parents
4. Birth Certificate issued by PSA of the minor
5. Valid Passport of minor (Data Page only)
6. Valid Residence Permit of minor
7. Valid Passport of travelling companion

IF APPLICABLE, BRING THE ORIGINAL AND COPY OF:

1. Dual Citizenship (Identification Certificate, Oath of Allegiance or Order of Approval)
2. Marriage Contract issued by PSA or if married in Spain the translated Certificado Literal de Matrimonio

APPLICATION PROCESS

1. Walk in to the Consulate (We are open Monday - Friday from 8:30am - 4:30pm except during declared PHL and ES public holidays)
2. Present your documentation at Window 1 or 2 
3. Pay at the Cashier (Window 3), payment by Cash, Debit/Credit cards and Bizum are accepted

Note: Personal appearance of the parent/s is required. The parents must sign all documents at the Consulate, in front of a Consular Official.
