REQUIREMENTS:

  1. NBI Application Form - to be given by the Philippine Consulate on the day of application
  2. Personal appearance of the Applicant
  3. Original and one (1) photocopy of the Data Page of the Philippine Passport
  4. Two (2) photos 5x5 centimeters size, taken in the last six (6) months with white background

In case you will need to Apostille your document/s at the DFA, present the additional requirements:

  1. Original and one (1) photocopy of the Data Page of the Philippine Passport
  2. Original Special Power of Attorney to be signed at the Consulate

APPLICATION PROCESS:

  1. Walk in to the Consulate (We are open Monday - Friday from 8:30am - 4:30pm except during declared PHL and ES public holidays)
  2. Present your documentation at Window 1 or 2 
  3. Pay at the Cashier (Window 3), payment by Cash, Debit/Credit cards and Bizum are accepted
