• Applicants can bring their own affidavit or they can download a template here
• Photocopy of valid identification card (passport)
• Other documents that the applicant would like to attach

APPLICATION PROCESS:

1. Walk in to the Consulate (We are open Monday - Friday from 8:30am - 4:30pm except during declared PHL and ES public holidays)
2. Present your documentation at Window 1 or 2 
3. Pay at the Cashier (Window 3), payment by Cash, Debit/Credit cards and Bizum are accepted

1. Personal appearance of the Applicant
2. Duly accomplished Application Form
3. Original and one (1) photocopy of the Data Page of the Philippine Passport
4. Three (3) passport photos 4.5 cm x 3.5cm in size, taken in the last six (6) months with white background

Note: In case the Passport was lost/stolen, the applicant will have to apply for an Affidavit of Lost Passport and submit it for the Travel Document Application.
